Policies and Statements

Board of Directors are required on a yearly basis to read and review the FSGA Ethics Policy, the Due Diligence policy and sign the Ethics Statement.  Please review the following forms, you may download them if you would like a copy.  Once you have read and reviewed, please sign below.  Your electronic signature will constitute agreement with these statements.  Please disclose any area of ongoing potential or appearance of conflict below.  These forms will be shared with the FSGA Executive Committee for review.

  • DISCLOSURE STATEMENT
    As an FSGA Directors or Officer, I affirm that I agree to act within accordance to FSGA policy regarding ethics, due diligence and disclosure. It is the policy of the Board of Directors of the Florida State Guardianship Association to assure that members of the Board are free from any negative influences caused by a conflict of interest. I recognize the duty to avoid self-dealing or conflicts of interest where, by reason of my position, I allow transactions to occur, which might not benefit the Association, and which give the appearance of or have the potential for conferring a benefit, monetary or otherwise, upon myself or my relatives, friends, or business associates. I have agreed to fully disclose any area of ongoing, potential or appearance of a conflict of interest. The following is a list of all areas of potential conflict of interest of which I am currently aware:
