2021-2022 Membership Application (Membership Year April 1, 2021 – March 31, 2022)
From October 1st thru December 31st Memberships will be Discounted. Reduced rate will be displayed on checkout page.
Dues
Individual Membership – $150
Any professional including professional and public guardians who serve as court appointed guardians and are registered with the Office of Public and Professional Guardians. Attorneys, fiduciaries, estate planners, court, clerk, medical professionals, social workers, etc.
Organizational Membership – $250
Includes 2 representatives, $50 for each additional representative
Individual Family/Volunteer Guardians – $75
Family members or friends who serve as a non-professional guardian for an individual
Optional Additional Fees
Chapters
Each Chapter has a separate fee and you can join as many chapters as you wish. You are not required to join a chapter but many members benefit from meeting regularly with local guardians and related professionals.
Member Designation & Website link
Your membership includes one designation for the FSGA Web Directory, $25 per additional designation. Add a link to your website to the directory for $25.
FSGA Supporters
Any donations are accepted. Donors with a donation of $10 or more will be recognized in the FSGA newsletter or on the website.
You may join online with a credit card, or download the application and mail it into our executive office with a check or credit card information.
